The program of all inclusive health care takes a thorough approach towards medication. During the PACE initial evaluation, participants are assessed for their ability to manage their medication before designing a personalized plan for them.
We make sure that the right medicines are ordered and delivered to the participant’s home either in vials or prepackaged sets for specific times during a day for the week.
Members who can independently manage their medication might not require a lot of help, while others might need reminders from a home health aid, nurses to administer the medication, or even access an automatic medication dispenser. In some cases, a nurse can also arrange a weekly pre-pour for some participants who need it.
