Zee Morales, one of our enrollment specialists at West PACE, visits with the host of “Oceanside Spectrum” on KOCT-TV to explain what a Program of All-Inclusive Care for the Elderly (PACE) program is and the services we provide to North County San Diego seniors and their family caregivers. PACE is a model of person-centered care that has been shown to reduce rates of emergency room visits, unnecessary hospital admissions, and long-term nursing home placements.
