PACE at home is designed to support your loved one’s ability to remain in their home by providing necessary medical and personal care where they are. Our trained Personal Care Attendants are ready to help when you cannot. In addition, our physical, occupational and speech therapists work tirelessly to support their overall well-being and help them recuperate after an illness or injury.
Getting Around
Are their driving skills the same as before? Do you feel safe when they drive you around? Are there any unexplained dents or scratches on the car?
How can West PACE help with transportation? We provide scheduled transportation that helps your loved one get to important appointments to stay healthy. We can also deliver medical supplies and fun activities to their door to save you the drive.
Social Life
Do they still engage in activities they once enjoyed? Are they reluctant to leave the house?

The House

How can West PACE help around the house? Our team can provide light housekeeping and help keep an eye on things around the house during those times when you can’t be there.
Meal Preparation & Nutrition
Is the refrigerator stocked with fresh foods? Is there moldy or expired food around? Are there burned pots and pans or burn marks on the floors or counters?
How can West PACE help with meal prep and nutrition? Our registered dieticians cater to participants and tailor meal plans to their dietary needs. This may involve substituting ingredients to avoid food allergies or medical conditions like diabetes while assuring meals taste great.
Medication Management
Are there any new medications, vitamins or supplements you haven’t seen before? Is their medication organized so it’s easy to take the correct dose at the correct time? Are expired medications mixed in with current ones?
How can West PACE help with medication management? We ensure participants enrolled in our care are on the right medications and provide assistance with taking them properly. We remove the stress of going to the pharmacy by delivering prescriptions to program participants’ doors.
